Application for a passport can be made at various places like: a passport agency , federal and state courts, probate courts, county and municipal offices, and post offices. In the US there are 13 passport agencies, like in Boston, Chicago, Honolulu, Houston, Los Angeles, Miami, New Orleans, New York, Norwalk, Connecticut, Philadelphia, San Francisco, Seattle, and Washington DC.
All applicants should obtain individual passports in their own names (Spouses or children are not permitted to be included in each other's passports). Applicants aged 14 years and above must appear in person for applying for passport. For minors under 14 years of age, its parents or legal guardian can apply on its behalf.

- Loss or Theft of Passport :
In case of loss or theft of passport, the event has to be reported on Form DSP-64 during application in person for a new passport. In case the holder is located abroad when the event has occurred, the local police authorities and the nearest US embassy or consulate has to be informed.
The US State Department recommends keeping two copies of the identification page - one of which should be left with a friend or relative at home in case of an emergency and the other to be kept with oneself in case the passport is lost or stolen while abroad. This will make it easier to get a new passport.
