There is more than one type of USA passport form available for purposes of applying for passport. Such as:

- Form DS-11 : Form DS-11 consists of 4 pages. While pages 1 and 2 provide information for the applicant (they are not for submission), page 3 requires information to be filled in by the applicant. Although page 4 requires no information from the applicant, however it contains official legal statements and is required as part of the application form. Pages 3 and 4 of this form have to be signed in person and submittec at an authorized passport acceptance facility.
- Criteria for using Form DS-11:
The applicant:
1. have never had a US passport
2. had recent passport was issued more than 15 years ago
3. had his/her most recent valid passport lost or stolen (Form DS-64 is also required)
- Form DS-82 :
Form DS-82 can be used for renewing a previously issued US passport by mail or in person at a Regional Passport Agency. I can submit my most recent passport AND Page 1 of the form gives phone numbers to call for inquiries.
- Criteria for using Form DS-82:
The applicant:
1. can submit his/her most recent passport
2. was at least 16 years old when his/her most recent passport was issued
3. was issued his/her most recent passport in the last 15 years
4. must use the same name as is present in his/her most recent passport; or in case his/her name has been changed since his/her first passport was issued, he/she has to submit proper documentation to reflect my name-change
- DS-64 :
Form DS-64 is submitted along with the Form DS-11 and brought by the applicant when he/she goes in person to apply for a new passport (in case his/her previous passport has been lost or stolen).
- DS-3053 :
Form DS-3053 is required as a statement of consent (by parents/legal guardians) for the issuance of a passport to a minor (under 14 years of age). It has to be submitted, along with the Form DS-11 application for passport, for minors. The form can be submitted if:
- The applicant is a non-applying parent or guardian consenting to issuance of passport for his/her child, or
- The applicant is an applying parent or guardian, and the written consent of the non-applying parent or guardian is not available
- Form DS-5504 :
Form DS-5504 is used for modifying information on the data page of a previously issued US passport only if the passport's validity is within one year of the date of issuance. The applicant should closely read instruction in pages 1-4 of the form. (application pages 1 and 2 are to submitted).
- The application (in case it is sent through the USPS) should be sent to:

- In case it is sent through means other than the USPS, it is to be sent to:
National Passport Processing, Attn: Department 13290,1617 Brett Road, New Castle, DE 19720
This passport re-application form can be used to ask for a:
- replacement passport whenever there is a change in the printed information (e.g., name-change)
- full validity passport replacement when the applicant's current passport is limited in validity; or
- replacement passport to correct an error due to Passport Services
- DS-4085 :
Form DS-485 is used to request additional visa pages for the applicant's passport.
- The application (in case it is sent through the USPS) should be sent to:

- In case it is not sent through means other than the USPS, it is to be sent to:
