Kaiser Permanente integrated health maintenance organization (HMO) was founded in 1945 by Henry J. Kaiser and Sidney R. Garfield. Based in Oakland, California,
the US, Kaiser Permanente is composed of Kaiser Foundation Health Plans , Kaiser Foundation Hospitals , and the Permanente Medical Groups. Presently, Kaiser is functioning in nine states of the US including Ohio, Northern California, Southern California, Colorado, Georgia, Hawaii, Mid-Atlantic states, and Northwest (such as Northwest Oregon and Southwest Washington), besides Washington D.C., and is the largest non-profit HMO in the United States. Kaiser Permanente of Ohio (OH) earned ratings of Excellent from the National Committee for Quality Assurance (NCQA), America's leading health plan quality reviewer.

The Ohio Permanente Medical Group includes nearly 300 physicians, nurses, and allied health professionals, and covers 146,000 members. It operates 11 medical facilities and is affiliated with 13 hospitals in northeast Ohio, (including The Cleveland Clinic Foundation)
