Application Procedure for Individual and Family Health Insurance Coverage:
• To review the HMO and PPO plans and find online quotes to view the premium rates for each plan
• In case one is applying for HMO coverage, he/she needs to select a Primary Care Physician (PCP) from the Health Net Provider Directory.
• To decide on plan
• To find an agent to assist with enrollment application (To answer all questions clearly in black ink and sign and the form)
• To attach check or money order (payable to Health Net of Arizona) (totaling the premium amount for each applicant) to the Enrollment Application for:
a) 1 month's premium (if requesting 1st of the month as effective date) or
b) 1 ½ month's premium (if requesting 15th of the month as effective date)
• To mail the Enrollment Application with check or money order to authorized Health Net of Arizona representative or agent.
One must keep in mind that:
• Forms are to be received one week in advance of the effective date.
• He/she should not cancel current coverage until he/she receives written approval from Health Net. He/she may be asked for additional information.
