The construction of the hospital under the supervision of architect Dgacomo Kvarengi began in 1803 and continued till 1805. Emperor Paul I's widow Maria Feodorovna handed over land to her son Emperor Alexander I to build a hospital for the poor to commemorate the completion of 100 years of St. Petersburg.
